Hepatitis and cirrhosis

The liver is the largest organ in the body, weighing up to 2.5 percent of total lean body mass.
 Located in the upper right quadrant of the abdomen, the liver varies in size and shape, depending on each person’s anatomy. 
Its main function is to metabolize substances in the blood in preparation for excretion, although it has many other important functions, including synthesis of most essential proteins, production of bile, and regulation of nutrients such as glucose, cholesterol, and amino acids.
The main kind of liver cell is called a hepatocyte. These cells comprise about two thirds of the liver’s mass. 
The liver’s blood supply comes from the hepatic artery, which supplies oxygen-rich blood. The liver also receives blood from the portal vein, which filters blood from the stomach, intestines, pancreas, and spleen.
The most common liver function tests are enzyme, bilirubin, albumin, and prothrombin time tests. The liver contains thousands of enzymes, only a few of which are routinely measured as indicators of liver function. These enzymes include the following:
Alkaline phosphatase. Abnormal levels may indicate bile obstruction, liver injury, or some forms of cancer.
Alanine transaminase. Abnormal levels may indicate hepatitis or other liver cell injury.
Aspartate transaminase. Abnormal levels may indicate injury to liver, heart, muscle, or brain.
Gamma-glutamyl transpeptidase. Abnormal levels may indicate organ damage, drug toxicity, alcohol abuse, or pancreatic disease.
Lactic dehydrogenase. Abnormal levels may indicate damage to liver, heart, or lung, and excessive breakdown of red blood cells.
5'-nucleotidase. Abnormal levels may indicate impaired bile flow.
The other major liver tests include the serum bilirubin test, which measures bile excretion, and the albumin test, which can indicate liver damage. Finally, the prothrombin time test measures the time needed for blood to clot. Because most blood clotting factors are produced in the liver, and they have rapid turnover, this test can help measure the liver’s ability to synthesize cells. 
Prothrombin may be elevated in hepatitis and cirrhosis as well as in disorders related to vitamin K deficiency.
Taken together, these tests provide physicians with a relatively complete picture of liver function and can help diagnose liver disease.

Cirrhosis Symptoms

Many people with cirrhosis have no symptoms during the early phases of the disease. Symptoms are caused by either of 2 problems:

1.Gradual failure of the liver to carry out its natural functions

2.Distortion of the liver's usual shape and size because of scarring

The symptoms of cirrhosis of the liver can vary greatly from patient to patient.

Hepatitis Risk

Intravenous drug use has been the greatest risk factor for hepatitis C since the early 1980s. Although transfused blood has been tested for both hepatitis B and C since the early nineties, individuals given transfusions before then, even decades before, may still be at risk. Such individuals are urged to be tested. Most health care providers are at low risk, although the chance of infection in hospital workers who are accidentally stuck with a needle is high, ranging from 4% to 10%.
Although heavy drinking itself is the major risk factor for alcoholic hepatitis, genetic factors may play a role in increasing a person's risk for alcoholic hepatitis. Women who abuse alcohol are at higher risk for alcoholic hepatitis and cirrhosis than are men who drink heavily. High fat diets may also increase the risk in heavy drinkers.
People who are at risk for developing hepatitis are workers in the health care professions, people with multiple sexual partners, intravenous drug users, and hemophiliacs. Hepatitis is generally thought to be as much as ten times more common in the lower socioeconomic and poorly educated groups. About one third of all cases of hepatitis come from an unknown or unidentifiable source. 
This means that you don't have to be in a high risk group in order to be infected with the hepatitis virus.
Feces-contaminated water and food are the major sources of infection of Hepatitis A , and infected people can transmit it to others if they do not take strict sanitary precautions. Hepatitis A is the hepatitis strain people are most likely to encounter in the course of international travel.
Up to 90% of Hepatitis B patients are men, although it can infect children.
Drug users who share needles are at considerable risk. Pregnant women with hepatitis B can transmit the virus to their babies. Contaminated medical instruments, including fingerstick devices used for more than one individual, have been known to transmit the virus.

Who hepatitis

Hepatitis is an inflammation of the liver, most commonly caused by a viral infection. There are five main hepatitis viruses, referred to as types A, B, C, D and E.

Hepatitis A and E are typically caused by ingestion of contaminated food or water.

 Hepatitis B, C and D usually occur as a result of parenteral contact with infected body fluids (e.g. from blood transfusions or invasive medical procedures using contaminated equipment). 

Hepatitis B is also transmitted by sexual contact.

The symptoms of hepatitis include jaundice (yellowing of the skin and eyes), dark urine, extreme fatigue, nausea, vomiting and abdominal pain.

Pegylated interferon(Pegasys, PEGIntron)

Pegylated interferon is a man-made form of interferon, a chemical naturally produced in the body to fight viruses. Interferon helps to stop the hepatitis C virus (HCV) from making new copies of itself. 


It also helps the immune system to kill HCV. Pegylated interferon is made with special add-on parts that help the drug stay in the body longer. It is injected once a week, while interferon that is not pegylated needs to be injected at least three times a week.

There are two brands of pegylated interferon: Pegasys and PEG-Intron. There are slight differences between the drugs, but both brands seem to work equally well and do not have major differences in side effects.

Hepatitis:

Hepatitis D is caused by the virus "HDV"You can only get hepatitis D if you are already infected with hepatitis B.

You get hepatitis E by drinking water infected with the virus.This type of hepatitis doesn't occur often in the United States.

Hepatitis A is caused by eating food and drinking water infected with a virus called "HAV".

Hepatitis B is spread through contact with an infected person's blood, semen or other body fluid.

Hepatitis C is spread the same way as hepatitis B, through contact with an infected person's blood, semen or other body fluid.

Hepatitis F

Hepatitis F is one of the major types of Hepatitis which affects

a large number of people all over the world. This is also known as

a hypothetical virus which is popularly linked to hepatitis.

The hepatitis F is one of the diseases which has spread mostly during

the 1990s. 

This is also known

as a nonexistent virus as well.

The Virus of the Hepatitis F is known to be Rounded 27-37 nm

and they

are also known as Virus-Like Particles (VLP). This is one

of the types of viruses which contain a double stranded DNA along

from stools of human patients. The virus was also seen

in the cytoplasm of hepatocytes.

What can cause chronic hepatitis?

Chronic hepatitis also has a number of different causes.

• Contagious viral hepatitis such as hepatitis B, hepatitis C and hepatitis D.
• Medicines.
• Toxins such as alcohol.
• Autoimmune hepatitis. This is a disease in which a number of liver cells are destroyed by the patient's own immune system. Autoimmune hepatitis can also sometimes occur as acute hepatitis. The cause is unknown.
• Inborn metabolic disorders, such as Wilson's disease (disorder of the body's copper metabolism) and haemochromatosis (disorder of the body's iron metabolism).

How do you get hepatitis?

A person can develop hepatitis if they contract one of the viruses that can cause liver inflammation, or as a result of exposure to substances that can cause hepatitis - alcohol, fungal toxins and certain medicines.

There are two ways in which medicines can lead to hepatitis: it can either occur as a result of medicine poisoning through overdoses of a medicine (eg paracetamol), or it can occur as a result of an abnormal reaction of the liver to a normal dose (eg halothane, the anaesthetic). Fortunately, the latter type of hepatitis is rare.
