Granulomatous hepatitis is a condition in which abnormal collections of white blood cells collect in the liver.

Fortunately, most people recover completely from hepatitis A, E and nonviral hepatitis. Mild flare-ups may occur over a period of several months with viral hepatitis. Each flare-up is usually less severe than the initial attack, and a relapse does not necessarily indicate that complete recovery will not take place.

Unfortunately, hepatitis B, C and D can linger in the body, producing chronic, perhaps lifelong, infection. Additionally, carriers of the hepatitis virus can infect others, even though they feel perfectly well. They may face risks of liver disease (cirrhosis and liver cancer) in the future.

Symptoms of Hepatitis

Hepatitis produces an initial �acute phase,� often with few if any symptoms. If there are symptoms, they tend to mimic "flu-like" symptoms such as:

mild fever
muscle or joint aches
nausea
vomiting
loss of appetite
slight abdominal pain
diarrhea
fatigue

The acute phase and its symptoms is rarely serious or fatal, although occasionally a so-called fulminant or rapidly progressing form leads to death.

Diagnosis of Hepatitis

The only approved treatment for hepatitis C virus, and the only one with demonstrated efficacy, is interferon alfa-2b (Intron A).

Currently, there is not effective treatment for hepatitis D and E.

For treatment of nonviral hepatitis, the doctor will first remove the harmful substance by flushing out the stomach via inducing vomiting or hyperventilation. If necessary, the patient with drug-induced hepatitis will be treated with corticosteroids.

It is the liver’s job to detoxify any substances that are potentially harmful to the body. An already damaged and weakened liver must work much harder than a healthy liver in order to accomplish this task. When a person with liver disease ingests a potentially hepatotoxic drug, this puts an additional strain on the liver and can result in further liver injury or possibly even liver failure. Even people with a healthy liver can develop liver disease as a consequence of ingesting a toxic medication or drug.

In general, people with liver disease should avoid medications known to be hepatotoxic. People who must be treated with a medication that is potentially hepatotoxic should have their LFTs closely monitored by their doctors. If a person’s LFTs become greater than three times baseline values, the medication causing these elevations should be discontinued. Amebic liver abscess is caused by Entamoeba histolytica, the same organism that causes amebiasis, an intestinal infection also called amebic dysentery. The organism is carried by the blood from the intestines to the liver.

Hepatitis D is a highly defective virus since it cannot produce infective virions without the help of a co-infecting helper virus. This helper virus is hepatitis B virus that supplies the HBsAg surface protein. In budding out of the cell, HDV acquires a membrane containing HBsAg. HDV is similar to a plant viroid in that it has a small circular RNA genome (1,700 bases) but unlike the plant viroids, the RNA encodes a protein called the delta antigen. This complexes with the RNA. The RNA is single stranded negative sense and is a covalently closed circle. Because of a large amount of base pairing, the RNA takes on a rod-like structure .

HDV can only form an infectious particle if the cell in which it replicates is co-infected with HBV since the latter provides the surface HBsAg which is required for reinfection of another cell. The HBsAg of HDV binds to the same surface receptor as HBV and the virus fuses with the cell membrane. The tropism of HDV is therefore the same as HBV. The RNA genome is coated with delta antigen, the only protein encoded by the RNA. The delta antigen, which is exposed when the envelope is lost, has a nuclear localization signal that targets the genome to the nucleus. Here the genome is copied by host cell RNA polymerase II, the enzyme that normally makes mRNA. Steatohepatitis resembles alcoholic hepatitis, but it can and does occur in people who seldom or never drink alcohol. In this instance, it is often called nonalcoholic steatohepatitis or NASH. Both alcoholic hepatitis and steatohepatitis can lead to serious liver damage and cirrhosis.

Studies have shown that many people who are significantly overweight have developed, or will develop, steatohepatitis. It can also occur with rapid weight loss. Steatohepatitis has been connected to estrogen hormones in some women. In the case of diabetes mellitus, researchers believe steatohepatitis may develop only in those patients whose diabetes is not properly controlled.

In most instances, treatment of fatty liver and steatohepatitis requires control of the underlying conditions. This may include reduction of high blood triglycerides, good control of diabetes, or not drinking alcohol. In some cases, surgical reversal of intestinal bypass for obesity is required.

Since being overweight is by far the most critical factor, weight loss is the key to ridding the liver of fat. This is especially necessary if damage to the liver is occurring, and early signs of scarring are present on biopsy. High blood triglycerides and diabetes are also worse with obesity. So, when steatohepatitis is present with these conditions, people gain even greater benefits from losing weight. Losing weight can be difficult. However, it must be done because the alternative may be eventual cirrhosis and the need for a liver transplant.

Currently, studies are underway on certain drugs such as Actigall. This drug appears to reduce liver damage in cases of steatohepatitis. At this time, however, it is not certain how helpful these drugs will be. To repeat the point, losing weight is by far the most important treatment.

Instead, the cancer in the liver is named for the organ or the tissue in which it began. For example, colon cancer that spreads to the liver is metastatic colon cancer. It is not liver cancer.

In the United States, metastatic cancer in the liver is far more common than primary liver cancer.

People with metastatic cancer in the liver have different treatment options than those with primary liver cancer.

Cancer begins in cells, the building blocks that make up tissues. Tissues make up the organs of the body.

Normal cells grow and divide to form new cells as the body needs them. When normal cells grow old or get damaged, they die, and new cells take their place.

Sometimes, this process goes wrong. New cells form when the body doesn't need them, and old or damaged cells don't die as they should. The buildup of extra cells often forms a mass of tissue called a growth, nodule, or tumor.

Growths in the liver can be benign (not cancer) or malignant (cancer). Benign tumors are not as harmful as malignant tumors:

Scientists from Greece have reported a possible link between liver damage similar to that caused by hepatitis and the consumption of herbal tea.

Writing in the July issue of the European Journal of Gastroenterology and Hepatology, lead author Spyros P. Dourakis from the University of Athens related the case of a 62-year-old man who was hospitalised after consuming a herbal tea made from Teucrium capitatum.

The man was hoping that the tea would help reduce cholesterol and high blood sugar levels, and had been taking it on a daily basis for four months before coming down with symptoms of nausea and loss of appetite. He stopped drinking the tea, but then developed jaundice and went to the hospital.

Dourakis and his colleagues discovered that the man was suffering from liver toxicity, and that the jaundice was similar to that caused by hepatitis. The patient eventually recovered from the symptoms, but only after a nine-week period.

"To the best of our knowledge, this is the first description of acute hepatitis related to the use of T. capitatum," Dourakis wrote in the journal. "Some medicinal plants may not be as safe as they are widely considered," he concluded, although adding that more evidence would be needed before a definitive statement could be made about the safety of T. capitatum.

There is much evidence to show the efficacy of herbal products in treating a range of maladies, but scientists are now increasingly looking into the potential adverse effects of the products as well. A report in The Lancet notes that despite past warnings about toxicity, comfrey products continue to be marketed as herbal teas, herb root powders, and capsules. Toxic levels of alkaloid have been found in commercial preparations.

Pyrrolizidine alkaloid poisoning is difficult to diagnose; it may be necessary to obtain and analyze herbal preparations used by the patient in order to discover the cause of liver failure. "Physician and consumer awareness of herbal toxicities is extremely limited, and the incidence of hepatic veno-occlusive disease and of pyrrolizidine poisoning may be grossly underestimated."

Unfortunately, a person cannot expect to walk into the doctor’s office and request “a diet for liver disease.”

Such an across-the-board diet simply does not exist. Many factors account for the unfeasibility of a standardized liver diet, including variations among the different types of liver disease (for example, alcoholic liver disease versus primary biliary cirrhosis) and the stage of the liver disease (for example, stable liver disease without much damage versus unstable decompensated cirrhosis).

One’s other medical disorders even if unrelated to their liver disease, such as diabetes or heart disease, must also be factored into any diet.

Each person has her own individual nutritional requirements, and these requirements may change over time.

Most people with liver disease find that eating multiple small meals throughout the day is the best approach, as it maximizes energy levels and the ability to digest and absorb food. Fatty liver is the collection of excessive amounts of triglycerides and other fats inside liver cells.

Also called steatosis, fatty liver can be a temporary or long-term condition, which is not harmful itself, but may indicate some other type of problem. Left untreated, it can contribute to other illnesses. It is usually reversible once the cause of the problem is diagnosed and corrected.

The liver is the organ responsible for changing fats eaten in the diet to types of fat that can be stored and used by the body. Triglycerides are one of the forms of fat stored by the body and used for energy and new cell formation. The break down of fats in the liver can be disrupted by alcoholism, malnutrition, pregnancy, or poisoning. In fatty liver, large droplets of fat, containing mostly triglycerides, collect within cells of the liver.

The condition is generally not painful and may go unnoticed for a long period of time. In severe cases, the liver can increase to over three times its normal size and may be painful and tender.

Fatty liver can be caused by certain chemical compounds, nutritional, or endocrine disorders. Drugs or chemical compounds that can cause fatty liver include alcohol, tetracycline, cortisone, phosphorous and carbon tetrachloride. Of these alcohol is by far the most common cause. Inflammation usually accompanies exposure to these toxins and is responsible for the associated symptoms of fever, fatigue and jaundice.

Nutritional causes of fat in the liver result from starvation, obesity, protein malnutrition and intestinal bypass operation for obesity. In obesity, the fatty deposits are accompanied by some inflammatory changes and mild scarring of the liver.

The endocrine causes of fatty liver include diabetes mellitus and fatty liver of pregnancy. In both cases, a large amount of fat can be rapidly deposited in the liver leading to expansion of the liver with tenderness in the upper right part of the abdomen. In diabetes this occurs only in juveniles. Fatty liver during pregnancy occurs near term and may result in premature termination of the pregnancy.

Fatty liver can be present in patients with alcoholic hepatitis and cirrhosis or may be an isolated finding.

Present evidence shows that while fatty liver is usually present in excessive alcohol intake, it probably does not lead to the development of alcoholic hepatitis or cirrhosis. In fact, most individuals with alcoholic fatty liver do not develop the more serious forms of liver disease.

However, continued alcohol ingestion has been known to cause alcoholic hepatitis and cirrhosis.

Often, there are no symptoms associated with fatty liver. If there are symptoms, they can include pain under the rib cage on the right side of the body, swelling of the abdomen, jaundice, and fever. Symptoms that occur less often in alcoholic fatty liver, but more often in pregnancy related fatty liver, are nausea, vomiting, loss of appetite, and abdominal pain.

Liver cancer is a disease involving uncontrolled growth of cells within the liver.

Cancer in the liver may be primary or secondary, also called metastatic. Primary cancer arises within the liver and in its early stages exists only within the liver. People who have hepatitis B or C, viral infections of the liver, or cirrhosis of the liver are more likely than other people to develop primary liver cancer. At an early stage primary liver cancer may cause no symptoms at all. More advanced disease may cause loss of appetite, weight loss, fever, fatigue and weakness.

Secondary liver cancer is term for cancer that originates in another organ, such as the colon, stomach, pancreas and breast and then spreads to the liver. Because secondary cancer is present in at least two organs, the treatment possibilities are more limited than for primary liver cancer. For years, scientists have warned people that having an apple-shaped figure or a beer belly is a health risk.

They said people with wide girths are more likely to have visceral or intra-abdominal fat, which increases their risk of diabetes, heart disease, stroke and some types of cancer.

But new research presented Sunday at the annual meeting of the Obesity Society shows that belly fat may not be the biggest bad guy behind some of the medical issues. "Fat in the liver is a more important indicator of health problems," says Samuel Klein, director of the Center for Human Nutrition at Washington University School of Medicine in St. Louis.

Klein and colleagues recruited 31 obese men and women and looked at their visceral fat, which surrounds the organs in the belly, and their liver fat. Some people had high amounts of liver fat; others had normal amounts. A normal liver contains 5% fat or less, but a severe fatty liver may contain up to 50%, Klein says. The latter is referred to as non-alcoholic fatty liver disease.

Using several different medical tests, the researchers found that people with fatty livers:

•Make more triglycerides, which are released from the liver into their bloodstream and can increase the risk of heart disease.

•Are more likely to be resistant to the action of their own insulin, meaning their bodies don't regulate blood sugar properly, which can lead to type 2 diabetes, Klein says. Over time, high sugar levels damage large and small blood vessels, leading to heart disease, stroke, nerve damage, amputations, blindness and kidney disease.

"We do not yet know whether the liver fat is causing these health problems or is simply a very good indicator of health risk," he says. "Even though it looks like visceral fat itself might not cause harm, it is often high in people with increased liver fat."

About 30% of adults and a third of overweight children and adolescents have too much liver fat, he says. If you are obese, 30 or more pounds over a healthy weight, you are at a higher risk of having non-alcoholic fatty liver disease.

The bottom line: If you're too heavy, it's a good idea to trim down, he says. A weight loss of as little as 2% to 5% can cause a marked reduction in liver fat.

In a related study, Elisa Fabbrini of Washington University School of Medicine found that severely obese people who lost weight after gastric bypass surgery significantly lowered the fat in the liver.

The first artificial organ for liver patients that uses immortalized human liver cells, the Extracorporeal Liver Assist Device, or ELAD®, is a bedside system that treats blood plasma, metabolizing toxins and synthesizing proteins just like a real liver does.

NewYork-Presbyterian Hospital/Columbia University Medical Center is currently one of only a small number of hospitals in the U.S. offering this therapy to acute liver failure patients as part of ongoing clinical trials.
"These studies are looking at how well the system can extend patients' lives until a liver transplant becomes available. We're also interested to see if it can relieve the burden on the patient's liver enough so that it can regenerate and regain some of its function," says Dr. Robert Brown, site principal investigator, chief of the Division of Abdominal Organ Transplantation, and director of the Center for Liver Disease and Transplantation at NewYork-Presbyterian Hospital/Columbia University Medical Center. Dr. Brown is also the Frank Cardile Professor of Medicine and Pediatrics (in Surgery) at Columbia University College of Physicians and Surgeons.
The ongoing studies look at whether ELAD liver support improves survival compared with standard medical therapy. Patients are randomly assigned to receive either standard medical therapy plus the ELAD system, or standard medical therapy alone. Patients eligible for the study have life-threatening acute liver failure, often due to an infection. Another trial open to patients with liver failure due to drug overdose without underlying liver disease is expected to begin enrollment later this year.
The current trials expand on prior results from Phase 1 and 2 trials in the U.S. and U.K., and a pivotal, randomized, controlled clinical trial at two sites in China during 2006 and 2007. In the latter study, 69 patients with hepatitis B or C who had suffered ALF were treated with either ELAD or standard therapy. Thirty-day transplant-free survival rates were statistically higher in the ELAD group compared with the control.
Artificial livers have been attempted since the 1960s. Because previous designs didn't use human liver cells, they couldn't adequately filter toxins or create chemicals essential to metabolism and blood-clotting.
With the ELAD system, four 12-inch cartridges containing cells derived from human liver cells and fibers are mounted on a standard blood-pumping unit. The patient's blood plasma flows inside of hollow fibers to allow appropriate two-way transfer of metabolites across the fiber membrane.
Liver transplantation is limited by the supply of donor livers. According to the United Network for Organ Sharing (UNOS), there were approximately 6,500 liver transplants performed in 2007; however, there are more than 16,000 patients on the waiting list. Each year only about one-third of those who need a donor liver will receive one, and many patients die while waiting.
Acute liver failure afflicts more than 30,000 Americans each year, including those with chronic liver diseases like hepatitis, as well as those whose livers were damaged, such as by taking too much acetaminophen pain medicine. If not treated effectively and promptly -- usually by transplantation -- patientsexperience organ failure, bleeding, coma and death. When a donor organ isn't available or if the patient is too sick for surgery, ELAD could be their only option.

Erythraea's bitter action stimulates digestive secretions; the secretion of gastric juice increases as soon as the herb acts on the mucous membranes of the mouth.
